DepEd, Division of Cebu City is now ACCEPTING TEACHER-APPLICANTS for School Year 2012-2013.

1.An applicant shall submit to the nearest elementary or secondary school head a written application supported with the following documents:

· CSC Form 212 (Revised 2005) in two copies with recent 2x2 ID picture

· Certified photocopy of PRC License (Date of validity should not be expired at the time of application)

· Certified photocopy of ratings obtained in the Licensure Examination for Teachers/Professional Board Examination for Teachers.

· PRC Certification as to the authenticity of eligibility

· Certified photocopy of transcript of records for baccalaureate course with computed weighted ratings



· Original copy of Certification of Residency, VOTERS ID / VOTERS AFFIDAVIT

· Certificate of specialized training, if any

· Certificate of specialized training, if any


2.All pertinent documents must be properly arranged following the above sequence and to be     placed in a white long folder with fastener.


3.All applicants are advised to be guided with the following schedule:

a) Acceptance of Application- January 16, 2012 to March 30, 2012

b) English Proficiency Test- April 3, 2012 (Zapatera Elementary School) to be administered by Intelliprime.  Applicants must register with Intelliprime prior to the test.

c) Demonstration and Interview- Inquire from the school where you have applied.

d) ICT Examination- April 9 to 13, 2012 (Register at the EDP Section of this office 2 to 3 weeks before the test, bring Certificate of ICTTraining / Certificate of Employment related to ICT)


4.Previous applicants who wish to retain their  points from last year’s ranking must submit a letter of request for retention with supporting documents directly to the school where they have applied. This Office will no longer retain previous points of applicants who failed to submit a letter of request.

6.The applicant assumes full responsibility and accountability on validity and authenticity of the    documents submitted. Any violation will merit corresponding sanctions after due process.
